    Our Programs

    We can’t do it alone.

    Our community benefit programs are organized to work collaboratively with community groups, schools and government organizations to effectively leverage resources and address identified community needs.

    The following is a small sample of the types of programs focused on meeting a variety of community needs. Additional programs can be found at each of our hospital and medical foundation Web sites. Links to the sites are located off Sutter Health’s home page.

    Youth Bridge Program
    The Youth Bridge Career and Internship Program at Alta Bates Summit Medical Center is a comprehensive, year-round youth development program designed to assist East Bay high school students by encouraging them to complete their schooling and to pursue higher academic and/or vocational goals.     Interim Care Program
    In partnership with the Salvation Army, The Effort, local health care systems and the County of Sacramento, Sutter Medical Center, Sacramento provides the Interim Care Program (ICP), a temporary respite program for homeless clients who are discharged from one of the participating hospitals.

    African American Community Health Advisory Committee
    The African-American Community Health Advisory Committee is a community partnership with Mills-Peninsula Health Services. The African-American Community Health Advisory Committee is dedicated to addressing issues and creating awareness of the major health concerns and issues affecting African-Americans, and to actively seek and promote a more healthful lifestyle.

    St. Luke’s HealthFirst
    HealthFirst, a center for prevention and education, is an innovative program offer through the St. Luke’s campus of California Pacific Medical Center focused on improving the health of uninsured and underinsured residents of San Francisco with chronic illnesses, such as asthma or diabetes. The HealthFirst team offers preventive care for high-risk patients by collaborating with these patients in the self-management of their chronic conditions.

    Healthy Connections
    Healthy Connections, located in downtown Tracy and supported by Sutter Tracy Community Hospital, offers a wealth of health education resources and social and family support services available through Sutter Tracy Community Hospital and county and local non-profit agencies.
